1. Health Inspections

This is the most heavily weighted category. It evaluates results from:

✔ Visit the facility in person. No rating can substitute for what you observe when you walk through the door. If you can, try to visit the facility unannounced. 

✔ Trust your instincts. If the facility feels chaotic, understaffed, or dismissive of your concerns, it probably is.
